What does this mean?
Hospitals are required by law to have consent from a parent or legal guardian in order to render treatment or procedures to children under the age of 18. If a child needs medical care and the parents are not present, the hospital must try to reach the parent before providing care.
What can you do as a parent?
Even though a parent cannot be with their children at all times, there is a way to make sure your child can receive the proper medical care when necessary. Whenever a child is left in the care of another responsible adult, an "Authorization for Consent to Medical treatment of Minor Child" form should be completed. These should be used for babysitters, camp supervisors, grandparents, etc. when the child is going to be away from the parents.
You never know when an accident may happen. The consent forms should be filled out completely. The appointed adult must bring the forms to the hospital should medical care be necessary.
What is Your Next Step
Enter your zip code to access an online consent form. Print and fill the form out, then provide a copy of the completed form to your child's caregiver(s).
