We know your time is valuable and this is one way we can ease the process. Online Registration will make your visit easier due to the fact all of the information will be in the department when you arrive at DeKalb Memorial Hospital. Is online registration secure? DeKalb patients will not have to sacrifice the safety of their personal information for this convenience. The online registration page is hosted on a secure server to ensure the privacy of DeKalb Memorial Hospital’s patients. The personal information provided is encrypted, and strong security measures are employed to protect and prevent the loss of patient information. Will the patient be required to stop at Registration? No, you can report directly to the point of service (X-Ray, Lab, Cardiac Rehab, Physical Therapy etc.) All necessary forms required will be completed at the point of service. You will only need to stop at the Patient Access Department if you are coming in for Labor and Delivery or Outpatient Surgery. May we also register by phone or fax? If you feel more comfortable completing the registration over the phone verses the internet, you can do so as well. You may call the Patient Access Department at 260.920.2603. Who do I contact for question or concerns? Registration is a function performed by the Patient Access Department at DeKalb Memorial Hospital. Please feel free to contact the Patient Access Department with any questions 260.920.2603. Will it be necessary for the patient to bring their insurance card? If you have new insurance or an updated card, you can stop by the registration department so we can scan your new card. However, this is not required.
